Central Community Health Centre (Central CHC) is offering community-based Advanced RN Footcare for Elgin County residents who do not have access to footcare services through a benefit plan or other funding source. The service is offered in partnership with the Thames Valley Family Health Team. Primary care providers can refer eligible patients, with priority given to individuals with diabetes mellitus with neuropathy, peripheral arterial disease, non-diabetic related foot neuropathy, or arthritic conditions. Providers are encouraged to include an updated cumulative patient profile, if available, and identify any service access challenges such as mobility or transportation barriers.

Provides publicly funded outpatient dietitian support in St. Thomas for eligible community members, including nutrition counselling relevant to diabetes, metabolic health and cardiovascular risk.
