The Elgin Primary Care Network (PCN) brings together family physicians, nurse practitioners and other primary care professionals across St. Thomas and Elgin County. By connecting local providers, sharing resources and strengthening collaboration, the PCN supports better coordinated care while ensuring primary care has a strong voice in local health system planning.
