Primary care providers can use the updated ParaMed Physiotherapy Clinic Patient Referral Form to refer patients for OHIP-funded or private physiotherapy services at the St. Thomas clinic. Closing the Gap has now been renamed to ParaMed. Provides government-funded clinic physiotherapy in St. Thomas for eligible patients, as well as rehabilitation and home-care services.

Provides Parkinson disease information, education, navigation and connections to regional support groups for patients and care partners.

Self-referral; patients, caregivers and providers may call the national information line.

Provides the physician referral form and patient information package for the regional Prostate Diagnostic Assessment Program; completed referrals are faxed to the program.
