Centralized referral pathway for ALS, epilepsy, first seizure, EMG, movement disorders, multiple sclerosis, neuromuscular medicine and neurosurgery.
Use the current specialty-specific form and criteria; referrals outside the listed central-intake services are declined and must go directly to the relevant provider.
