Central Community Health Centre (Central CHC) is offering community-based Advanced RN Footcare for Elgin County residents who do not have access to footcare services through a benefit plan or other funding source. The service is offered in partnership with the Thames Valley Family Health Team. Primary care providers can refer eligible patients, with priority given to individuals with diabetes mellitus with neuropathy, peripheral arterial disease, non-diabetic related foot neuropathy, or arthritic conditions. Providers are encouraged to include an updated cumulative patient profile, if available, and identify any service access challenges such as mobility or transportation barriers.

Provides evidence-based, stigma-aware primary-care recommendations using the 5As framework for assessment and individualized behavioural, pharmacologic and surgical obesity management. Guide with all the nutrition services offered by registered dietitians

Provides a consistent digital endocrinology referral structure in the provincial eReferral network to support complete clinical information, appropriate triage and referral tracking.

Integrates fracture and fall-risk assessment, repeat BMD timing and treatment decision support into OSCAR, Accuro and TELUS PS Suite EMRs.

Provides primary-care algorithms, a quick-reference guide and treatment resources for fracture-risk assessment, BMD testing, pharmacotherapy, monitoring, exercise and nutrition.

Provides access to the Canada-specific FRAX fracture-risk calculator, CAROC and other tools supporting osteoporosis assessment and treatment decisions.

Provides a dedicated regional referral pathway for assessment and management of confirmed or suspected thyroid cancer through the endocrine program.
